The book 'Combat Squadrons of the Air Forces, World War II', edited by Maurer Maurer, USAF Historical Division, Air University, Department of the Air Force, 1969 (U.S. Government Printing Office) pp.684-686, detail the squadron emblems and approvals (for the 612, 163, 614 and 615.
I have used library copies for my reference work. I don't know if new copies are available for sale, but some used military book stores may know (or have copies).
