Can anyone tell me how to get a deceased veterans service records? What information do I need and who to contact.

Hi Fred,

You can write to:

National Personnel Records Center
Military Personnel Records
9700 Page Avenue
St. Louis, MO 63132-5100

Request a complete 201 file, including all awards and medals. I did have a web link to download the form but I have misplaced it. I wrote to a friend that works there and ask him to tell me again how to get the form. I will let you know as soon as I have that information.

The form you need is the SF 180 form.

Ps: Many people do not know they can fax the form. They mail it and because they get thousands of requests a day...it helps to find any way to help speed up the process even a little.

Make sure you request a copy of any awards/medals that person received as a result of service. They are provided free of charge. I just got a set that my dad was awarded.
